Pharmacodynamics and pharmacokinetics

The active ingredient is vancomycin.

The drug has a bacteriostatic effect on enterococcus and acts bactericidal on other microorganisms. The main component of the antibacterial drug is produced by Amycolatopsis orientalis. The main mechanism of action is aimed at blocking the process of synthesizing the wall of a bacterial cell, which ultimately leads to cell lysis. The active substance is able to affect the permeability of the membranes of bacterial cells, selectively inhibit the process of RNA synthesis.

Edicin affects corinobacteria, staphylococci, enterococci, actinomycetes, clostridia. The drug acts on strains that are resistant to penicillin.

Optimum efficiency is observed at a pH of 8; a sharp decrease in the effectiveness of the drug is observed when the pH is reduced to 6. The drug has an active effect only on those microorganisms that are in the stage of reproduction. Mycobacteria, gram-negative flora, protozoa, viruses and fungi are resistant to the action of the medication.

Cross-resistance with other antibacterial drugs is not observed.

Indications for use

Instructions for use Edicin recommends prescribing a medication for pneumonia, sepsis, meningitis, endocarditis, lung abscess, pseudomembranous colitis, skin infections, enterocolitis, osteomyelitis and other infectious lesions of the joints and bone tissue.

Contraindications

Edicin is not used for breastfeeding, auditory nerve neuritis, gestation, intolerance to the active component of the drug.

With pathology of the renal system, impaired auditory perception, the medication is prescribed after consulting specialists, with caution.

Side effects

Urinary system: level up urea nitrogen and creatinine, nephrotoxicity (possible development of renal failure) with simultaneous treatment with aminoglycosides, rarely develops interstitial nephritis.

Digestive tract: pseudomembranous colitis, nausea.

Sensory organs: vertigo, hearing impairment, ototoxicity, tinnitus.

Hematopoietic organs: rarely develops agranulocytosistransient thrombocytopenia, reversible neutropenia.

In violation of the norms and rules of infusion, local reactions develop in the form of pain at the injection site, phlebitis, tissue necrosis at the site of infusion.

Allergic reactions rarely develop: chills, nausea, febrile syndrome, exfoliative dermatitis, rash, eosinophilia, vasculitis, epidermal toxic necrolysis, exudative malignant erythema.

With rapid administration, post-infusion reactions develop: syndrome "red man"caused by the release histamine (spasm of the muscles of the back and chest, hyperemia of the skin of the face and skin of the upper half of the body, fever, chills, palpitations), anaphylactoid reactions (itching, dyspnea, bronchospasm, drop in blood pressure, skin rashes).

Edicin, instructions for use (Method and dosage)

Edicin is administered only intravenously, the infusion rate is not more than 10 mg per minute, the duration of infusion is not more than 1 hour. Intramuscular injections are extremely painful.

Adults are prescribed 7.5 mg / kg every 6 hours or 15 mg / kg every 12 hours. For patients with pathology of the excretory function of the renal system, the drug is prescribed in an initial dosage of 15 mg / kg, further tactics of vancomycin therapy are determined by the QC indicator (when registering uremia, injections are carried out with an interval of 10 days).

Before the introduction of the solution, the powder is dissolved in water for injection: 1 gram in 20 ml (solution concentration 5 mg / ml). The prepared solution is recommended to be diluted with a solution of dextrose 5% or sodium chloride 0.9%: 500 mg - 100 ml.

Treatment of pseudomembranous colitis: Edicin is taken orally as a solution: 0.5-2 grams are diluted in 30 ml of water, taken 3-4 times a day. The course of antibiotic therapy is 7-10 days.

Overdose

It is manifested by an increase in the severity of the above adverse reactions.

The treatment is asymptomatic in combination with hemofiltration and hemoperfusion.

Interaction

In the treatment cholestyramine there is a decrease in the effectiveness of edicin. Phenothiazines, meclosin, antihistamines, thioxanthenes can mask symptomatic manifestations ototoxicity Edicin (vertigo, tinnitus).

During treatment with vecuronium bromide and the use of general anesthetics, neuromuscular blockade may develop, and blood pressure levels may decrease.

The introduction of vancomycin is possible no earlier than 1 hour after the use of vecuronium bromide and general anesthetics.
